About this Resource

This blood pressure self-monitoring diary is designed to assist your clients in tracking their blood pressure over time. It is generally recommended to take at least 16 blood pressure readings (4 each day for 4 days) and to take the average of both readings (diastolic and systolic) at the end to calculate baseline blood pressure. This can then be used to monitor any changes over time.

“Self-measured blood pressure (SMBP) monitoring refers to the measurement of BP by a patient at home or outside of a clinic setting. … The American Heart Association recommends SMBP measurements for evaluation of most patients with known or suspected hypertension to assess response to treatment and possibly improve adherence.”
